CPAP Service Exchange Request 2018
Normal service is recommended, but not required, every 2 years. Operating conditions, volumes and geographic considerations vary from user to user. Therefore, users should continually monitor the effectiveness of their units and when in doubt, contact us.
After purchase is completed, an exchange Unit will be shipped, along with a return label (pre-paid) and packing instructions. Its as simple as placing the unit in need of repair into the same bubble wrap and box received, applying label, and dropping at your Post Office or with your mail delivery person (*pending they can accept packages in the field).
** By purchasing this service ticket, I recognize that if I have damaged parts that are beyond typical “service” wear and tear (i.e. major damage, missing significant missing pieces, etc.), I will be contacted in the event the unit returned in the exchange is not to in salvageable condition for the exchange program.
By Purchasing this service ticket I acknowledge and agree to the terms identified in the preceding paragraph and authorize Emergent Respiratory to begin the process.
